Senior Member Application
The official application procedure is as follows:
1. Obtain a Preliminary Application Form from the state society Corresponding Secretary who is Carol Leavitt by writing, calling or emailing her at:

2. Complete and submit the Preliminary Application Form and the latest Application Fee to the state society Corresponding Secretary.
o All names in the lineage are to be full names, for females use maiden surnames.
3. The state society Historian will send you an Application Worksheet with at as filled out as is possible with the Historians sources.
4. Complete the Application Worksheet sent to you by the state society Historian which has the very latest information on as many generations of your line as we have in our records.
o Mark and attach all proof of lineage documentation as instructed
o Submit to the state Historian who will check the Application Worksheet and when it is complete to the satisfaction of the Historian an Application form will be completed for you to attach all proof of lineage documentation and sign.
o The state society Historian will forward Application to the General Society of Mayflower Descendant for their approval
5. .When approved by the Historian General a membership certificate, termed a Passport, will be sent to the state Historian for presentation to the new member.
